How to use dental insurance at Kālos
It’s super simple to apply for reimbursement on orthodontic services. If your dental insurance covers orthodontics, after purchase, just log into your providers portal or follow the steps below:
1Fill out claim form
Select a provider to download their dental claim form.
Don’t see your provider?
Contact your insurance company directly for instructions on applying for reimbursement.
Print your Kālos receipt. You can find your receipt in the email received when your purchase was made, or you can also request a receipt at our retail stores.
3Submit claim form and receipt to your provider’s address
4Get money back
After submitting your claim, you’ll typically be reimbursed within 2-3 weeks. (Depending on your provider, it can be faster or slower.) The best part: Most insurance plans will cover most of your Kālos purchase!
